EAB's dedicated client services team clinical staff make working with our laboratory easy and stress-free for you, your staff, and your patients.  It is easy to get started.

Open an Account - Complete online Account Setup form to open your account. 

Test Preparation Instructions - Give patient the Test Prep Booklet at least 4 days prior to blood draw to allow for any medication avoidance that may be necessary.

Review Phlebotomy Instructions - If you do not have a phlebotomist, we can provide your patients with a national referral service. 

Choose the test panel to be performed

Fill out the Requisition Form completely - Make sure test is marked and payment is included.
